Ready to start getting your private practice credentialed with insurance companies? It can be a long and tedious process. To make it as easy as possible, have these documents ready.
Information you should gather before you start credentialing:
  1. Locate your CAQH and NPI account information, if you do not have those accounts, you will need to create them
  2. Clinician areas of specialty and treatment modalities
  3. Current resume
  4. Service locations, remittance address, billing contact information
  5. List of specialty trainings and certification; CEU trainings for the past 12 months
  6. Liability insurance with a minimum of $1,000,000/occurrence and $3,000,000/aggregate coverage
  7. Know if your office location is ADA compliant
  8. Previously credentialed provider numbers for example with Medicare and Medicaid, if applicable
